Video and Photographic Release Form

    I hereby grant the Deviation Games LLC, its directors, officers, employees, agents, and designees (collectively “DG”) non-revocable permission to capture my image, voice and likeness in photographs, videotapes, motion pictures, audio recordings, or any other media (collectively “Images”). I acknowledge that DG will own such Images and further grant the DG permission to copyright, display, publish, distribute, use, modify, print and reprint such Images in any manner whatsoever related to DG business, including without limitation, publications, advertisements, brochures, web site images, or other electronic displays and transmissions thereof. I further waive any right to inspect or approve the use of the Image by the DG prior to its use. I forever release and hold the DG harmless from any and all liability arising out of the use of the Images in any manner or media whatsoever, and waive any and all claims and causes of action relating to use of the Images, including without limitation, claims for invasion of privacy rights or publicity. 

    Photographic, audio or video recordings may be used for ANY USE which may include but is not limited to: 
    • Presentations; 
    • Courses; 
    • Online/Internet Videos; 
    • Media; 
    • News (Press);
      By signing this release, I understand this permission signifies that photographic or video recordings of me will be electronically displayed via the Internet or in the public educational setting.
       
      There is no time limit on the validity of this release nor is there any geographic limitation on where these materials may be distributed.
       
      By typing my name and electronically signing this release, I acknowledge that I have completely read and fully understand the above release and agree to be bound thereby. I hereby release any and all claims against any person or organization utilizing this material for educational and business purposes. 
    By typing my name and clicking submit, I am legally agreeing to this video release as written above.
